Alderon’s model is being built around a consistent theme in the research: virtual care is most useful when it is integrated with an ongoing clinical relationship, a structured transition plan, and clear pathways for local testing or in-person escalation when needed.

Primary Care Continuity

Telemedicine can improve timeliness while continuity still matters.

A 2024 study of more than 4.8 million primary-care visits found telemedicine visits were generally timelier. The study also found that differences in short-term return office and emergency-department use were smaller when telemedicine was delivered by the patient’s own primary-care clinician rather than a less familiar clinician—supporting the value of continuity rather than disconnected virtual encounters.

Hospital-to-Home Transitions

Structured transitional care can reduce avoidable utilization.

A systematic review and network meta-analysis of 126 randomized trials involving more than 97,000 participants found that transitional-care interventions were associated with reductions in hospital readmissions at multiple follow-up intervals. Some intervention types also improved medication adherence and reduced emergency-department use.

Patients Without Established Primary Care

Post-discharge access may matter most when no usual clinician is available.

A systematic review focused specifically on patients discharged without an established primary-care provider. Evidence was limited and heterogeneous, but four of the five included studies reported reductions in emergency-department visits or readmissions among patients seen in dedicated transitional-care clinics.

Connected Care

Home measurements are most useful when paired with clinical support.

A 2024 systematic review and meta-analysis of 65 articles involving more than 21,000 participants found home blood- pressure monitoring improved blood-pressure control compared with usual care. The effect was stronger when home monitoring was paired with telemonitoring or additional clinical support—consistent with using data to inform care rather than simply collecting it.
