Alderon Medical Primary Care Programs

Focused care for the health concerns patients are actually facing.

Alderon’s planned primary-care programs are being built for adults age 18 and older to give selected health concerns more focused attention without separating them from the rest of primary care. Weight, menopause, men’s health, recovery, sexual health, sleep, connected health, and healthy aging can each have a deeper clinical pathway while remaining part of one longitudinal relationship.

  • Evidence-informed
  • Designed to stay connected to primary care
  • Built for longitudinal follow-through

The Alderon Approach

Primary care programs should make primary care stronger—not fragment it.

A patient may eventually come to Alderon because of weight, menopause, fatigue, sexual health, recovery, or another focused concern. That concern still exists within a larger health picture. Alderon’s planned primary-care programs are designed to preserve that context.

What that is designed to mean in practice

  • Review medications in the context of the whole patient
  • Connect focused treatment to prevention and chronic care
  • Use testing when it answers a clinical question
  • Coordinate local evaluation when telehealth is not enough
  • Reassess treatment over time rather than stopping at the prescription
  • Avoid building care around hype, arbitrary targets, or one-size-fits-all protocols

Programs + Primary Care

You should not need a different clinic for every part of your health.

Once enrollment opens, one of Alderon’s focused programs may be the reason a patient first enters care, but the relationship is designed to extend beyond that single concern. When appropriate, focused treatment can connect naturally to ongoing primary care, prevention, chronic-disease management, medication review, and future health needs.

Learn more: The Agency for Healthcare Research and Quality describes high-quality primary care as whole-person, integrated, accessible care built around continuity, coordination, and comprehensive health needs.

At launch, connected care may include:

  • Primary and preventive care
  • Chronic-condition management
  • Medication reconciliation
  • Laboratory and screening coordination
  • Same-day or transitional care when appropriate
  • Longitudinal follow-up as health needs change
