The value is in what happens between the visits, too.
A good ongoing care relationship keeps track of medications, screening, prior results, chronic conditions, new concerns, outside specialists, and what should happen next. That continuity gives each visit more context.

Start with the whole picture
Review medical history, medications, preventive needs, current symptoms, outside care, and what matters most to the patient right now.
Address what can be managed here
Many common health concerns, chronic conditions, medication questions, and preventive needs can be evaluated and followed within an ongoing care relationship.
Coordinate what belongs elsewhere
Local examination, laboratory testing, imaging, procedures, emergency care, or specialty consultation can be added when the clinical situation requires it.
Follow through
Results, medication changes, referrals, preventive needs, and new diagnoses matter most when someone is responsible for keeping track of the next step.
What Primary Care May Include
Everyday health needs with a longer view.
Alderon is designed to manage common health needs while recognizing when testing, examination, specialty care, or a higher level of care should be added.
Screening & health maintenance
Review age-appropriate screening, vaccinations, cardiovascular risk, metabolic health, and other preventive priorities.
Medication management
Review treatment response, side effects, interactions, adherence, refill needs, and whether a medication still fits the current care plan.
Ongoing condition management
Follow common chronic health conditions with appropriate laboratory data, home measurements, medication adjustments, and risk reduction.
Evaluation of new concerns
Begin the evaluation of appropriate new symptoms and determine whether virtual management, local testing, examination, or another level of care is appropriate.
Specialist & outside-care follow-up
Help connect recommendations, outside records, diagnostic testing, referrals, and treatment changes back into the broader care plan.
Long-term risk reduction
Address smoking, weight, sleep, nutrition, physical activity, cardiovascular risk, metabolic risk, and other modifiable contributors to future health.
Continuity of Care
A familiar clinical history can change the quality of the next decision.
When medications, prior results, chronic conditions, prevention needs, and previous treatment decisions are already part of the relationship, a new concern can be evaluated with more context.
Continuity also creates a place for results to return, referrals to be revisited, and treatment plans to be adjusted as health needs evolve.
Ongoing care can connect
- Preventive screening and health maintenance
- Medication reconciliation and follow-up
- Laboratory and home-health trends
- Chronic-condition management
- Specialist recommendations and outside records
- New symptoms and changing health priorities
Telehealth-First Care
Virtual when it works. Local when it matters.
Many routine follow-up visits, medication reviews, preventive discussions, chronic-condition visits, result reviews, and selected new concerns work well through telehealth.
When services launch and an examination, laboratory test, imaging study, procedure, or specialty assessment is clinically important, Alderon can help coordinate that step locally rather than pretending every problem belongs on a screen.
Your care may include:
- Secure virtual visits
- Local laboratory coordination
- Imaging when clinically indicated
- Home vital signs when useful
- Specialty referral and coordination
- In-person evaluation when necessary
When Virtual Care Is Not Enough
Good care includes recognizing the limits of telehealth.
Some symptoms need urgent or in-person evaluation.
Chest pain, severe difficulty breathing, stroke symptoms, severe abdominal pain, major bleeding, significant neurologic changes, severe allergic reactions, loss of consciousness, or another potentially serious condition should not wait for a routine virtual visit. Call 911 or seek appropriate emergency care when indicated.

One relationship can support different kinds of care over time.
Frequently Asked Questions
Primary care at Alderon, explained.
Can Alderon be my regular primary-care practice?
Alderon is being built for an ongoing care relationship rather than isolated one-time visits. Once services launch, the exact care available will depend on the patient’s location, clinical needs, and whether virtual care is appropriate for the problem being addressed.
What happens if I need an examination?
Alderon can identify when an in-person examination is clinically important and help direct the next step. Some problems simply cannot be evaluated safely through telehealth alone.
Can Alderon order laboratory testing?
Once services launch, clinically appropriate laboratory testing may be coordinated locally and reviewed as part of the patient’s ongoing care plan.
Can Alderon coordinate referrals?
Once services launch, when specialty evaluation, imaging, procedures, or another level of care is appropriate, referral and coordination may become part of the treatment plan.
Is every health problem appropriate for telehealth?
No. Alderon uses telehealth when it is clinically appropriate and directs patients toward local, urgent, emergency, or specialty care when the situation requires something different.
Can focused Alderon programs still connect back to my regular care?
Yes. Focused programs are designed to remain connected to medications, prevention, chronic conditions, laboratory data, and the broader longitudinal care relationship.
