Alderon’s model is being built around a consistent theme in the research: virtual care is most useful when it is integrated with an ongoing clinical relationship, a structured transition plan, and clear pathways for local testing or in-person escalation when needed.
Telemedicine can improve timeliness while continuity still matters.
A 2024 study of more than 4.8 million primary-care visits found telemedicine visits were generally timelier. The study also found that differences in short-term return office and emergency-department use were smaller when telemedicine was delivered by the patient’s own primary-care clinician rather than a less familiar clinician—supporting the value of continuity rather than disconnected virtual encounters.

A systematic review and network meta-analysis of 126 randomized trials involving more than 97,000 participants found that transitional-care interventions were associated with reductions in hospital readmissions at multiple follow-up intervals. Some intervention types also improved medication adherence and reduced emergency-department use.

A 2024 systematic review and meta-analysis of 65 articles involving more than 21,000 participants found home blood- pressure monitoring improved blood-pressure control compared with usual care.
